2012 NYSDA New Dentist Leadership Award

The New York State Dental Association is inviting its components to submit the name of one deserving candidate who meets the criteria below to take home its New Dentist Leadership Award. All nominations will be considered by the NYSDA Council on Membership & Communications and New Dentist Committee.

The winner of the Leadership Award will receive $500 and a plaque of honor at the annual NYSDA House of Delegates meeting.

The 2011 NYSDA New Dentist Leadership Award recipient was Ruben Cohen, D.D.S., a member of the New York County Dental Society.

Guidelines

Nominations to include:

• Nominee’s current curriculum vitae.

• A single-page essay documenting the nominee’s leadership qualities, professional accomplishments and volunteer activities at all three levels (national, state, component) of organized dentistry and community service involvement. Include specific examples.
• A letter of recommendation from the president of the nominee’s component society.

Eligibility requirements:

Component entries must meet the following eligibility requirements:

• Each component may nominate one candidate and nominee must have the endorsement of his/her component dental society.
• Nominee must be a NYSDA member in good standing.
• Nominee must be less than 10 years out of dental school.

Nominations must be postmarked by September 30, 2012.
