CE Registry Template

Components and dental meetings must submit CE information electronically to the NYSDA Registry.

• In lieu of the aforementioned electronic template, dental meetings may submit to the Registry a CD-rom containing all CE course file information, including list of registrants with ADA numbers, course title or subject code, date of presentation and number of credit hours.
