X-Ray Machine Registration Renewal in NYC  

The NYC Department of Consumer Affairs sent a letter notifying dentists in New York City of a new requirement to renew their x-ray machine permit. When it is time to renew the permit within the 2-year cycle beginning June 1, 2017, dentists must include a copy of the summary page of their last inspection (form RC43 or RC45). This is to ensure that they have been inspected within the previous 5 years. Those dentists who were not given a copy of their last inspection, or cannot find it, can call the NYC Department of Health at: (347) 396-6000 with their record/camis/accela number and ask for the summary sheet.
